You can also request application material by mail. Contact the graduate secretary in the Mathematics Department. Make sure you include your name, complete mailing address, program you are interested in, whether you are U.S. citizen or international student, and when you would like to enroll. The University of Iowa General Catalog is also on-line.
CHECKLIST FOR APPLYING TO DEPARTMENT OF MATHEMATICS GRADUATE PROGRAM
1) Complete the application form of the Graduate College (downloadable-see above) (Original to Admissions - Copy to Department of Mathematics)
2) Complete the Information on Courses Sheet (downloadable-see above) (Submit to Department of Mathematics Only)
3) Print three Letter of Recommendation forms (downloadable-see above) (These forms should be completed by present or former professors who can recommend you for graduate study in Mathematics)
4) Official Transcripts, GRE and TOEFL scores are required by both Admissions and the Department of Mathematics
