You must also download/complete the following Supplemental Forms required by the Math Department:
1) LETTER OF RECOMMENDATION FORM and
2) INFORMATION ON COURSEWORK FORM.
You can also request application material sent to you by mail. If so, you can
CHECKLIST FOR APPLYING TO DEPARTMENT OF MATHEMATICS GRADUATE PROGRAM
1) Complete the online graduate admission application form of the Graduate College (downloadable-see above) and submit to Admissions Only. (If you are NOT applying online, mail graduate admission application forms to both Admissions and to the Department of Mathematics.)
2) Complete the Information on Coursework (downloadable-see above) and submit to the Department of Mathematics Only.
3) Print three Letter of Recommendation forms (downloadable-see above). These forms should be completed by present or former professors who can recommend you for graduate study in Mathematics and the letters sent by them directly to the Department of Mathematics Only.
4) Official Transcripts are required by both Admissions and the Department of Mathematics.
5) GRE General Test scores are required by Admissions Only.
6) For international students: TOEFL scores are required by Admissions Only.
Note: The form "Application for Graduate Awards" from the Graduate College is NOT required anymore. Every applicant will be considered for an award.
